Q: What’s the overall process to get me certified with the Michigan Medical Marijuana Program?
A: You need to apply in writing to the Michigan Department of Community Health with the following: 1) A patient application, 2) A physician certification signed by a physician, 3) A caregiver application signed by your caregiver (if applicable), and 4) A check or money order to the State of Michigan MMMP for their fee. These forms are available to download from the State of Michigan, as linked on our website, or we will provide them at the time of your appointment.

Q: How do I obtain my physician certification?
A: Assuming you have a qualifying condition, you first gather the last 12 months of your medical records from your MD or DO and fax or mail them to us. Medical records must be current within the last 3 years. Once reviewed, we will contact you, so please include your name, phone number, and best time to reach you on a cover sheet. If an appointment is made, after review of your records, you will come to our office for your appointment with Dr. Bix.

Q: What happens at my appointment?
A: During your appointment, you will complete a set of new patient paperwork, including a review of systems and a pain survey. Once your paperwork is complete, you will be examined by the doctor, who will discuss your options and make recommendations based on your history and objective findings. If you are qualified, your physician certification would be completed and copied for you.

Q: What do I need to bring?
A: 1) A government issued photo ID, 2) A payment method such as cash or money order. We can accept Visa/MC only in our Troy office, 3) Copies of any additional medical records that were not sent previously, such as Xray’s or further chart notes.

Q: What happens after my appointment?
A: Your paperwork will be assembled in an envelope for you to mail to the State, minus your check or money order payable to the State. You will get an extra copy of your packet to hang onto, while you wait for your card. You will send the packet by certified mail to the State of Michigan Medical Marijuana Program. The State will issue you a Patient Registration Card, which allows you to possess, use, and grow Cannabis.

Q: How much does it cost?
A: Our office charges $200 for initial certifications. The State of Michigan charges $100. So the overall costs would be $300 initially. The registration is renewed annually, with charges subject to change.

Q: What sorts of discounts are available?
A: The State allows SSI, SSD, Full Medicaid recipients a discount of $75. So instead of mailing them a check for $100, it would be $25, and you would send them proof of your benefits. You should refer directly to their website for more information about the specifics regarding their discount.

Q: What if I don’t have medical records?
A: We won’t be able to assist you any further with regard to medical marijuana. We would suggest you get a primary care physician. You may inquire with us regarding primary care.

Q: What about chiropractor records?
A: In addition to MD or DO records, they may help substantiate your history, but alone they are not enough.

Q: Do you do primary care?
A: We currently have a busy General Medicine practice, but do take on new patients in some circumstances.

Q: What if I have pending legal issues?
A: Very rarely would we certify a patient with pending legal issues. If you have issues, be upfront about it and talk with us before your appointment as there are rare exceptions. The program is not intended to circumvent the legal system or otherwise undermine the law.
